All students who participate in athletics must have a current physical on file with the LCA Sports Medicine Staff. KRS 156.070 (2)(d) states: “Every local board of education shall require an annual medical examination performed and signed by a physician, physician assistant, advanced practice registered nurse, or chiropractor (if performed within the professional's scope of practice), for each student seeking eligibility to participate in any school athletic activity or sport."
UPDATE: Physicals are now valid for exactly 13months, i.e. June 1, 2014 through July 1st, 2015. This makes it easier to plan and get annual physicals done and have insurance approve the visits (insurance usually only allow one physical per year).
The physical must be obtained prior to participating in any practices, scrimmages, or games. All completed physical forms and associated forms should be submitted to the athletic training room or LCA athletic office. A copy of this physical form may be found here:
Each year, physicals are performed at the school for all students needing a physical. Doctors from around the area are gracious enough to come and support our athletic program and take time out of their busy schedule. These physicals are not mandatory, however it is an easy way for student-athletes to be sure they are up to date on their physicals.
